LifeRx Program

A yearlong lifestyle change initiative proven to help prevent and delay the onset of type 2 diabetes, LifeRx encourages participants to make lasting behavioral changes involving nutrition, physical activity and coping strategies. Classes are instructed by a trained lifestyle coach.

This program is for pre-diabetics and those with specific risk factors. It does not treat or provide information for those already diagnosed with Type 1 or Type 2 diabetes. If you have Type 1 or Type 2 diabetes, talk to your primary care provider to learn about other services such as the diabetes care center, that may be right for you.

Service Area
  • Chickasaw Nation Residents - This service is available inside the Chickasaw Nation's boundaries.
  • Chickasaws At-Large - This service is available outside the Chickasaw Nation's boundaries.
Eligibility
  • 18 years of age or older
  • BMI (body mass index) of 24 or higher
  • Must have one or more of the following:
    • Positive screening for prediabetes based on the CDC Prediabetes Screening Test
    • History of gestational diabetes mellitus (diabetes while pregnant)
    • Prediabetes diagnosis confirmed by a blood glucose test within the past 12 months
