1. Visit the MyChart sign-up page.
2. Enter your activation code and date of birth.
3. Create your username and password.
4. Complete account setup and start using Chickasaw Nation Health MyChart.

1. Visit the MyChart sign-up page and select “Sign up without an activation code.”
2. Provide your personal information (name, date of birth, contact details).
3. If the info matches our records, you will be prompted to set up two-factor authentication for the phone number on file.
4. Once verified, account setup is complete.

• From the Chickasaw Nation Health MyChart homepage, choose Schedule an Appointment.
• Select a provider from the care team or use the “Tell us why you are coming” option to get matched with a provider.
• Choose the visit type and preferred date/time.
• Select the visit format.
• Review and confirm the new appointment.
• To reschedule or cancel, go to Upcoming Visits and select the appointment you want to change. (Note: Cancellation timing restrictions may apply depending on visit type and clinic.)

Update personal info anytime in MyChart under Personal Information. Under Settings > Personal Information > Details About Me > Edit, you can update your information.

For medical record errors, notify the provider at your next visit.

After sending a message, MyChart displays a confirmation, and you can view sent messages anytime under the Messages tab.

MyChart messages are for nonemergency, routine health questions. Urgent issues should always be addressed by calling 911 in an emergency. For mental health emergencies, call or text 988 to reach the Suicide & Crisis Lifeline.

1. Log in to your Chickasaw Nation MyChart account.
2. Open the Menu and search for "Link My Accounts."
3. Follow the prompts to connect accounts from other participating health care organizations.

Yes:

• Ages 0-15: Parents/legal guardians have full access.
• Ages 16-17: Parents/legal guardians have limited access. Teens can grant full access by request.
• Ages 18+: Parental access ends automatically. Adults must grant access through their own MyChart account.

Parental access ends automatically when a child turns 18. The adult child must regrant access via MyChart.

Use "Share My Record" in the Sharing Hub. They will need a record in our system first.

Yes, the proxy will need to have their own Chickasaw Nation Health MyChart account.

If they are not eligible yet, they will need to work with the front desk to get a record created and set up their MyChart access.

Visit the Chickasaw Nation Health MyChart password reset page, click Forgot Password? and follow the prompts. You will verify your identity before creating a new password.

Visit the Chickasaw Nation Health MyChart username recovery page. Click Forgot Username? on the login page and follow the verification steps to retrieve it.

Go to Settings > Personal Information > Communication Preferences to select how to receive messages (text, email, phone). Messages will not include sensitive health info.
